A Typical Problem with Conventional Systems

I am sure that every physician has had this typical experience in the office (albeit with different tests).

A patient is scheduled for an annual exam. You quickly glance over the patient’s chart and refer to your last office notes. The patient had a previous abnormal mammogram, which the radiologist recommended a repeated test in 3 months. There are no further notes on the chart.

Entering the room, you ask the patient if she had the follow-up mammogram completed. Looking confused, she says, “No. Why – was it abnormal?” There’s an immediate feeling of fear from both you and your patient. Questions run through your mind – did I note the results and my staff didn’t follow up? Was my staff unable to contact the patient? Did I ever see the original abnormal mammogram results?

What happened is irrelevant – this was an error and you are responsible.

My first attempt at eliminating this potentially devastating error in the future was an index card tickler file.
